Founded by Jasmin, a social worker with over 10 years of experience in social work, mental health, and the NDIS sector, Elevate Care Connections is built on the belief that every individual is unique, and so is their journey. Jasmin’s experience spans across both local and international settings, where she’s worked in community care, mental health, AOD (Alcohol and Other Drugs), and education. Her passion is centred around helping people achieve their personal goals, and she is committed to providing the highest level of care and support to NDIS participants and their families.
